MT ZION PERFORMING ARTS WAIVER (REQUIRED)

I give Mt. Zion and its assigns the irrevocable right to use graphic images such as photographs, video, and digital pictures of the enrolled student taken in past, and future gatherings, rehearsals, performances, on and off the premises for advertisement and instructional purposes. Advertisements include, but are not limited to, print media, web, and social networks such as Facebook and Twitter. I hereby release and agree to hold harmless Mt. Zion and its assigns from any liability by virtue of any blurring, distortion, alteration, or use in composite form whether intentional or otherwise that may occur in the taking or processing of the photographs. I hereby assume responsibility, and release Mt. Zion, its employees, sponsoring organizations, etc. from all responsibility for my child if he/she leaves the building for any purpose I hereby assume all responsibility for myself and for my children while participating in activities sponsored by Mt. Zion. I acknowledge that participation in the activities offered by Mt. Zion Performing Arts involves risk to the participant (and to the participant’s parents or guardians, if the participant is a minor), and may result in various types of injury including, but not limited to, the following: sickness, exposure to infectious/communicable disease, bodily injury, death, emotional injury, personal injury, property damage, and financial damage. I waive all claims for liability against Mt. Zion, its employees/instructors, its assigns in case of injury. I release and promise to indemnify, defend, and hold Mt. Zion harmless for any injury arising directly or indirectly out of participation in Mt. Zion Performing Arts. I commit my child to performing in all rehearsals and shows that are required for the Mid-Year Spotlight and May performance. I have read, understand, and agree to all the policies and procedures including payment terms as well as refunds/cancellations for registered classes as found in the published Registration Booklet at Mt. PAYMENT INFORMATION

I understand to confirm my registration and class enrollment Mt. Zion Performing Arts must receive my total payment due within 10 business days. Choose between the options below to submit payment:

Payment Plan Desired (Choose One) *

Full Year in Advance – Tuition in full is paid at time of registration. Costumes and other fees are due at 2nd and 3rd billing. There is a one-time $15 discount offered per family.
Three Term Payment Plan – One third of full season tuition is due at time of registration with the balance of tuition, costume and others fees split between 2nd and 3rd billing.
